1- Retrograde menstruation

One of the most accepted theories is that the blood that needs to be thrown out during menstruation goes back to the tubes from the uterus and spreads from there to the abdominal cavity, intestines and ovaries. It is thought that the living endometrial cells in this blood settle in the organ where they go and continue to grow there.

The fact that endometriosis is more common in those with heavy menstrual bleeding, prolonged and frequent menstruation supports this theory.

However, studies have shown that almost all women bleed backwards, but chocolate cysts do not form in all of these women. For this reason, it is thought that the underlying genetic predisposition plays an important role in the development of chocolate cyst.

In Which Age Group Is Chocolate Cyst Most Common?

Chocolate cyst is mostly seen in women of reproductive age between the ages of 15-45. It is not seen before puberty and after menopause. Chocolate cyst is an estrogen dependent disease.

Since the body estrogen decreases before puberty and after menopause, it regresses even if there is a disease. In girls who have painful menstruation in adolescence, endometriosis may be encountered in 8-10 years in advancing ages.

The aim of the hormonal treatment of chocolate cyst is to prevent the bleeding of the endometriotic tissue by preventing the hormonal stimulation in the menstrual cycle. However, it should be known that hormone therapy is only a suppressive treatment. If the hormone drug is stopped, the disease will recur. Therefore, hormonal drugs should be used for a long time.

1. Birth control pills: Birth control pills are a good option in cases of mild endometriosis. It should be used for at least 6-12 months. 2. Progesterone-containing drugs: Progestin-containing drugs are used in the treatment of endometriosis through the same mechanism as birth control pills. These drugs are taken by mouth daily. Open Endometriosis Surgery

Closed chocolate cyst surgery is performed with a laparoscopy camera with 20 times magnification and by creating an air chamber in the abdomen with CO2 gas. In this way, the surgical field is more clearly visible, adhesions are more easily separated, and less damage is done to the ovarian tissue.

The aim of closed chocolate cyst surgery is the same as open chocolate cyst surgery. Endometriosis foci in the abdomen, uterus and ovaries are removed and burned one by one. Intra-abdominal adhesions are removed. If there is a chocolate cyst, it is removed together with the cyst capsule. The color of the fluid collected in the cystic structure is pending blood and is popularly called chocolate cyst because of its dark brown color and appearance resembling melted chocolate.

What should be the approach in women with only cysts and no other complaints?

In this type of patient group, the best treatment approach would be to keep the cyst under observation for a certain period of time without resorting to surgical intervention. If there is an elevated Ca125 value, which is a tumor marker in blood tests, or if the diameter of the chocolate cyst exceeds 4-5 cm, surgical intervention may be decided in patients.

Prior to laparoscopic surgery, the ovarian reserve capacity of the patient should be evaluated by counting the ovarian sacs by ultrasound and measuring AMH (antimüllerian hormone). If it is determined that the woman's ovarian capacity is low as a result of this evaluation and if the woman is childless, the surgical intervention should be avoided as much as possible.

In patients with low ovarian reserve, we may recommend direct in vitro fertilization if they want to have children. In patients in this situation, Ca125 measurements should be performed at 3-6 month intervals. In women who have children or who do not want to have children in the future, laparoscopic surgical removal of the cyst should be performed.

What should be done in women with chocolate cysts and inability to conceive?

In these patients, ovarian reserve should be evaluated first. If it is determined that this reserve is sufficient, if the cyst is unilateral, laparoscopic surgery and the tubes are healthy, the woman should be expected to conceive spontaneously for 6-12 months depending on the woman's age.

In women with poor ovarian reserve, especially in women over 38 years of age or if the cyst is bilateral, the best treatment approach is direct IVF treatment. It is recommended not to enter the cyst during egg collection during IVF treatment. If this is not observed, i.e. if a needle is inserted into the cyst, there may be an increased risk of groin infection (pelvic infection and abscess) and ovarian abscess.

There is no consensus on which treatment methods should be applied to women with chocolate cysts who have repeated failures in IVF treatment.

Surgical removal of the chocolate cyst may be recommended for women with three or more failures in IVF. In this group of patients, spontaneous pregnancy has been achieved in 50% of cases after laparoscopy.

What should be done for women who still have cysts after repeated surgeries?

Complications of laparoscopic surgery are high in such patient groups. If the patient does not complain of pain, then close follow-up is recommended. In women who do not want to have children and complain of pain, the uterus and ovaries may be removed.

If patients want to have children, in vitro fertilization treatment can be applied. However, the tubes of the patients should be evaluated before this treatment. Because there may be adhesions that may occur after repeated surgical interventions and blockage in the tubes after the tubes are affected by endometriosis.

The tubes may have a sausage-like appearance and may be filled with endometriosis tissue. This factor can reduce the woman's chances of pregnancy even with IVF. When a blockage in the tubes is detected, it may need to be removed laparoscopically or closed in the area adjacent to the uterus.

If laparoscopy carries a high risk for patients, then hysteroscopic sterilization techniques can be used.

What is Adenomyosis?

Adenomyosis is when the endometriosis tissue that forms the inner wall of the uterus is located in the muscle tissue of the uterus. It gives symptoms by bleeding into the muscle tissue during each menstrual period. It becomes more frequent with increasing female age. Adenomyosis can be focal, i.e. located in one area of the uterus, or diffuse, i.e. diffusely involving the uterus.

Adenomyosis adheres to the uterine wall in a radial way and it is very difficult to separate from the uterine myometrium tissue because it is not encapsulated like fibroids. On ultrasound examination, it can be compared with the appearance of fibroids in the uterus.

What are the Symptoms of Adenomyosis?

The most important symptom of adenomyosis is severe menstrual and non-menstrual groin pain. Bleeding irregularities, prolonged bleeding and difficulty in conception may also be experienced. There may be complaints of pain during sexual intercourse. Sometimes adenomyosis can develop without any symptoms. Adenomyosis can sometimes give similar symptoms and be confused with fibroids.

How is adenomyosis treated?

Hormones and painkillers used in the treatment of chocolate cysts can be used in the treatment of adenomyosis. Hormonal spirals are also effective in the treatment of adenomyosis. Hormone medication or hormonal spiral is useful in reducing the amount of bleeding and pain

Surgical alternative may be considered in patients who do not respond to medication or hormonal spiral. Laparoscopic surgery is recommended for focal adenomyosis in the uterus, while open surgery is preferred in cases of widespread adenomyosis in the uterus.

The uterus can be removed in women at the end of reproductive age. Patients who have difficulty conceiving may be offered IVF treatment before surgery.

Recovery process after chocolate surgery

If endometriosis surgery is performed laparoscopically, the patient can stand up within 4-6 hours and be discharged within 24 hours after surgery. These patients can return to daily life or work in 1 week. If open surgery is performed, hospitalization may take 2 days and return to work may take up to 2 weeks.

In endometriosis surgery, if repair of the intestinal wall has been performed, it is recommended that the patient be monitored in the hospital for 5 to 7 days for intestinal leakage control.

If ureter or bladder repair is performed in endometriosis surgery, the patient can be discharged with a bladder catheter or ureter catheter after urine leakage control with postoperative imaging techniques. The bladder catheter is removed after 1 week and the ureter catheter after 3 or 6 months.

In endometriosis surgery, if a certain part of the intestine is removed (segmental resection) and end-to-end joining is performed, an ileostomy (opening of the intestine into the abdomen) can be opened to the patient as a protective measure. This is preferred if the level of the intestine area joined to the end is close to the anus.

In this method, which reduces the risk of leakage and increases patient safety, defecation is performed through the ileostomy bag attached to the abdominal wall. After 3 months, the intestinal segment in the abdominal wall is taken into the abdomen with the surgery to be performed and the intestinal segment that is joined end-to-end and healed starts to work.

Pregnancy After Endometriosis Surgery

Pregnancy plan after chocolate cyst surgery is made according to the findings obtained during surgery. If the tubes are found to be in good condition and the patient is not of advanced age, natural pregnancy can be tried for 3 to 6 months 2 months after the operation. If the condition of the tubes in chocolate cyst surgery is unhealthy or if the tubes need to be removed, IVF treatment can be applied 2 months after surgery.

After pregnancy occurs, a negative development in the pregnancy process is rare. Acute abdominal pain caused by leaks from chocolate cysts may be encountered rarely.

Sexuality after Endometriosis Surgery

There is no restriction on sexuality after chocolate cyst surgery. The patient can have sexual intercourse when her general condition improves after surgery. Since different pain thresholds of patients play a role in this period, no exact time is given.

Can chocolate cyst be treated with medication?

Chocolate cyst disease can be suppressed with medication and the complaints caused by the disease can be reduced. However, there is no definitive treatment for endometriosis with medication. The disease continues throughout the reproductive age with the support of the ovarian estrogen hormone.

Can the uterus be removed in chocolate cyst disease?

In chocolate cyst disease, if there have been recurrent surgical processes and the patient has completed reproduction, the decision to remove the uterus and ovaries may be made. When making this decision, the risks of osteoporosis and cardiologic risks due to estrogen deficiency should be evaluated.
